The Callendoc 'Dynamic Forms' feature lets you build any screen you want, without restrictions. Creating forms out of the elements you choose, and saving what goes into them for later use, is now very straightforward.
In short, we have made your screen designs something you control.
You can create patient satisfaction surveys, specialty-specific clinical screens and much more; use this module to design screens containing exactly the fields you want.
Detailed analysis of the information entered is possible too.
1. Creating a form
Start by giving your form a name on the Add Dynamic Form screen.

2. Building the content
Choose the screen elements for the form you have created. The following options are available.
- Page Note: an information note where you can explain what the page is for.
- Heading: headings you can use to group elements on the form.
- Line: a horizontal rule you can add to separate groups on the screen.
- Select: the section where you define the options available on the form.
- Text Box: where you create a single-line text field.
- Hidden: lets you create a hidden object on the form.
- Password: for confidential entries such as passwords.
- Text Area: defines multi-line text entry fields.
- Radio: the control that lets you pick only one of the options on a form.
- Check: the control that lets you pick several options on a form.
- Multi Select: lets you specify whether multiple selections are allowed and how they are made.
- Date: lets you create a date field.
That is all there is to it...
Every form you build will now appear on your consultation screens.
